At Reform Health + Wellness, we believe healthcare fees should be clear and easy to understand. Below you will find information about our treatment fees, extended health benefits and direct billing options.
Not every extended health benefits plan allows for direct billing. Please check with your insurance provider if direct billing is allowed or if you need to self-submit your receipt to your insurance provider.
Chiropractic
Our chiropractors provide individualized care for a wide range of musculoskeletal concerns, including back pain, neck pain, headaches, joint stiffness, sports injuries, and movement-related issues. Treatment may include chiropractic adjustments, soft tissue therapy, acupuncture or dry needling, shockwave therapy, mobility work, and exercise-based rehabilitation when appropriate. Your care plan is based on your assessment, goals, preferences, and response to treatment, with a focus on improving comfort, function, and confidence in movement.

Custom Orthotics
Custom orthotics are individually designed to support your feet, improve comfort, and help address concerns such as plantar fasciitis, foot pain, and lower-limb discomfort. At Reform Health + Wellness, we use a clinical assessment and digital 3D foot scanning to create custom orthotics tailored to your foot shape, activity level, footwear, and treatment goals.

Massage Therapy
Our Registered Massage Therapists provide individualized treatment to help address muscle tension, soreness, stiffness, stress, and movement-related concerns. Appointments may include a combination of therapeutic massage, deep tissue techniques, relaxation-focused treatment, stretching, cupping, or other hands-on techniques based on your needs and preferences. Whether you’re managing an injury, recovering from activity, dealing with persistent muscle tension, or simply looking to relax, your treatment will be tailored to your goals and comfort level.

Shockwave Therapy
Shockwave therapy is a non-invasive treatment that uses acoustic pressure waves and may be considered for certain persistent muscle, tendon, and soft-tissue concerns. Treatment is targeted to the area being assessed and is often used alongside exercise, mobility work, chiropractic care or other rehabilitation strategies when appropriate. Your practitioner will determine whether shockwave therapy is suitable for your condition, discuss what to expect, and tailor treatment to your symptoms, goals, and comfort level.

Acupuncture / IMS / Dry Needling
Acupuncture, IMS, and dry needling are needle-based techniques that may be used to help address muscle tension, pain, stiffness, and movement-related concerns. Treatment is tailored to your assessment and may involve targeting specific muscles, trigger points, or areas of sensitivity to help improve comfort and function. These techniques are often combined with chiropractic care, exercise, mobility work, or other therapies when appropriate. Your practitioner will discuss which approach may be best suited to your symptoms, goals, and comfort level.

Counselling / Therapy
Our counselling and therapy services provide a welcoming, supportive, and confidential space to navigate emotional, behavioural, and mental health challenges. Our team of psychology and psychotherapy professionals offers personalized, evidence-informed care for concerns such as anxiety, depression, stress, burnout, trauma, ADHD, relationship difficulties, emotional regulation, chronic pain, and life transitions. Depending on your practitioner and individual needs, treatment may incorporate approaches such as cognitive behavioural therapy (CBT), acceptance and commitment therapy (ACT), EMDR, and other therapeutic methods tailored to your goals. Psychological assessments for concerns such as ADHD and learning difficulties may also be available through qualified psychology providers. Whether you’re working through a specific challenge or looking to improve your overall emotional well-being, our team is here to help.

Direct Billing
We are pleased to offer direct billing to most major extended health insurance providers.
Depending on your plan, your insurer may pay all or part of your appointment fee directly to the clinic. Any remaining balance is due at the time of your appointment.
To help us submit your claim, please bring or upload:
- Your insurance provider and policy information
- Your member or certificate number
- The policyholder’s name and date of birth
- Information for a secondary insurance plan, when applicable
Direct billing is offered as a convenience but cannot be guaranteed. Insurance companies determine whether a claim is eligible, how much is covered and whether additional documentation is required.
When a claim cannot be submitted or is declined, you will be responsible for paying the appointment fee. We will provide a detailed receipt that you can submit to your insurer for possible reimbursement.
Understanding Your Coverage
Every insurance policy is different. Before your appointment, we recommend confirming:
- Whether your selected healthcare service is covered
- Your annual coverage limit
- The percentage or maximum amount covered per visit
- Whether a physician’s referral is required
- Whether your plan permits direct billing
- Whether you have reached your annual maximum
Our administrative team can assist with submitting claims, but insurance providers generally do not give clinics complete access to the details of a patient’s benefit plan. Patients remain responsible for understanding their own coverage.
Payment and Receipts
Any portion of your appointment fee that is not covered by insurance is payable at the time of your visit.
A detailed receipt will be emailed to you through Jane following payment. Printed receipts are also available upon request.
Gift cards may be purchased online and used toward eligible clinic services. Gift cards cannot be submitted to an insurance provider or exchanged for cash.
Cancellation Policy
Appointments are reserved specifically for you. We require at least 24 hours’ notice when cancelling or changing an appointment.
Our current policy is:
- First occurrence: Courtesy warning
- Subsequent occurrence: Fee equal to 50% of the scheduled appointment
Late-cancellation and missed-appointment fees cannot be billed to extended health insurance.
Frequently Asked Questions
Yes. We can submit claims directly to most major extended health insurance providers. Direct billing availability depends on the insurer, the healthcare service and your individual benefit plan.
Coverage varies. Some plans cover the entire appointment, while others cover a percentage or a fixed amount. You are responsible for any balance that is not paid by your insurer.
A referral is not generally required to book an appointment at Reform. However, some insurance policies require a physician’s referral before they will reimburse services such as massage therapy. Please confirm your plan’s requirements directly with your insurer.
You will be asked to pay for the appointment. We will provide a detailed receipt that you can submit to your insurance company.
We may receive basic claim information when submitting an appointment, but we cannot guarantee access to your annual limit, remaining balance or complete policy details. Your insurer can provide the most accurate information.
Coverage depends on your plan, the profession providing the treatment and how the service is billed. Please contact your insurer before treatment when coverage is important to you.
