At Reform Health + Wellness, we believe healthcare fees should be clear and easy to understand. Below you will find information about our treatment fees, extended health benefits and direct billing options.

Chiropractic Fees

Prenatal chiropractic appointments are available at our standard chiropractic rates.

An initial assessment is required before booking follow-up care. Patients returning with a new concern or after an extended absence may be asked to book a re-examination.

Massage Therapy Fees

Massage treatments may include therapeutic, deep-tissue or relaxation techniques based on your goals, preferences and the clinical judgment of your Registered Massage Therapist.

Acupuncture, IMS & Dry Needling Fees

Not every extended health plan includes acupuncture or dry needling coverage. Please check your individual plan before your appointment.

Shockwave Therapy and Additional Services

Shockwave therapy fees and appointment options can be viewed through our online booking system.

Fees for physiotherapy, psychology and other new services will be added as appointments become available.

Direct Billing

We are pleased to offer direct billing to most major extended health insurance providers.

Depending on your plan, your insurer may pay all or part of your appointment fee directly to the clinic. Any remaining balance is due at the time of your appointment.

To help us submit your claim, please bring or upload:

  • Your insurance provider and policy information
  • Your member or certificate number
  • The policyholder’s name and date of birth
  • Information for a secondary insurance plan, when applicable

Direct billing is offered as a convenience but cannot be guaranteed. Insurance companies determine whether a claim is eligible, how much is covered and whether additional documentation is required.

When a claim cannot be submitted or is declined, you will be responsible for paying the appointment fee. We will provide a detailed receipt that you can submit to your insurer for possible reimbursement.

Understanding Your Coverage

Every insurance policy is different. Before your appointment, we recommend confirming:

  • Whether your selected healthcare service is covered
  • Your annual coverage limit
  • The percentage or maximum amount covered per visit
  • Whether a physician’s referral is required
  • Whether your plan permits direct billing
  • Whether you have reached your annual maximum

Our administrative team can assist with submitting claims, but insurance providers generally do not give clinics complete access to the details of a patient’s benefit plan. Patients remain responsible for understanding their own coverage.

Payment and Receipts

Any portion of your appointment fee that is not covered by insurance is payable at the time of your visit.


A detailed receipt will be emailed to you through Jane following payment. Printed receipts are also available upon request.


Gift cards may be purchased online and used toward eligible clinic services. Gift cards cannot be submitted to an insurance provider or exchanged for cash.

Cancellation Policy

Appointments are reserved specifically for you. We require at least 24 hours’ notice when cancelling or changing an appointment.

Our current policy is:

  • First occurrence: Courtesy warning
  • Subsequent occurrence: Fee equal to 50% of the scheduled appointment

Late-cancellation and missed-appointment fees cannot be billed to extended health insurance.

Frequently Asked Questions

Yes. We can submit claims directly to most major extended health insurance providers. Direct billing availability depends on the insurer, the healthcare service and your individual benefit plan.

Coverage varies. Some plans cover the entire appointment, while others cover a percentage or a fixed amount. You are responsible for any balance that is not paid by your insurer.

A referral is not generally required to book an appointment at Reform. However, some insurance policies require a physician’s referral before they will reimburse services such as massage therapy. Please confirm your plan’s requirements directly with your insurer.

You will be asked to pay for the appointment. We will provide a detailed receipt that you can submit to your insurance company.

We may receive basic claim information when submitting an appointment, but we cannot guarantee access to your annual limit, remaining balance or complete policy details. Your insurer can provide the most accurate information.

Coverage depends on your plan, the profession providing the treatment and how the service is billed. Please contact your insurer before treatment when coverage is important to you.
